The injured worker sustained an industrial injury with cumulative trauma from 2/21/2016, to 8/11/2017, affecting his left shoulder, internal and cardiac systems, neck, left arm, cervical spine, right knee, left knee, lumbar spine, right heel and foot, as well as causing stress, strain, anxiety, depression, and a hernia.
